This position is part of the CN Guidance Critical Time Intervention Team and focuses on delivering evidence-based, time-limited, phase-based care management for individuals experiencing transitional care challenges.
Job DefinitionWork with the CN Guidance Critical Time Intervention Team to deliver services within an evidence-based model that is time limited, phase-based care management approach focused on enhancing continuity of care during transitional times. The Mental Health Professional resolves clinical and rehabilitative needs to meet the individual's recovery goals.
- Sign-on bonus of $5,000 provided you remain employed in this role full-time for 6 months.

- Licensed Creative Arts Therapist
- Limited permit permissible
Two years of experience working with the chronically mentally ill (Masters level).
Duties and Responsibilities- Responsible for providing Assessment and Service Planning, Quality Assurance and Improvement Processes, Utilization Review, incident reporting and coordination between CTI and other systems.
- Provide culturally sensitive and developmentally appropriate treatment to individuals and their families. Services include assertive engagement, case management, assessment, family counseling/therapy, family psychoeducation, individual psychoeducation, psychosocial rehabilitation, crisis intervention services.
- Maintain case records and complete documentation on time in compliance with federal, state and agency regulations.
- Provide direct, face-to-face and/or collateral services to recipients either in office or in the community.
- Must be available on an as-needed basis 24/7, including after-hours coverage on the phone and in person as needed.
- Assist recipients in developing appropriate expectations from treatment.
- Exchange information with other agencies that benefits the recipient or ensures compliance with regulations and coordination of services.
- Utilize and understand the value of supervision to increase practice skills, promote better services for the recipient and preserve accountability to the Agency.
- Participate in staff development, provide staff supervision and training.
- Actively engage in agency-wide trainings and work groups as well as NYS-specific training on the CTI model.
- Facilitate sharing of data with the individuals to whom it applies to facilitate partnered decision-making and keep clients informed of progress.
- Complete QI training during onboarding and participate in QI refresher training as needed.

- Must be willing to transport clients in an agency vehicle on an as-needed basis.
