Position SummaryThe Clinical Pharmacist is responsible for coordinating day‑to‑day provision of high quality, comprehensive pharmaceutical care to patients in accordance with established regulations and procedures. Some drug distribution duties will be required. Frye Regional Medical Center is dedicated to making communities healthier and being a champion of patient care.
Essential Functions- Collaborate with physicians and other healthcare professionals to optimize medication therapy for patients.
- Complete critical patient monitoring and reviews patient profile/chart to identify, prevent, or mitigate drug‑related problems, including interpreting laboratory data and recommending changes as needed.
- Document all clinical activities and interventions accurately and completely.
- Partner as an active member of multidisciplinary team and collaborate with healthcare providers within decentralized patient care areas to provide patient centered care.
- Participate in the management of medical emergencies.
- Provide patient counseling as appropriate.
- Dedicate at least 50% of time on antimicrobial stewardship.
- Participate in codes.
- Serve as a member on at least one hospital committee.
- Initiate or participate in quality improvement projects and recommend updates/modifications as necessary to optimize pharmacy services.
- Coordinate pharmacotherapy activities and education opportunities for department pharmacists and other healthcare professionals.
- Maintain proficiency in preceptor roles and actively emulate education/mentorship skills.
- Maintain proficiency and actively engage in central dispensing functions and operations programs.
- Monitor drug therapy regimens for accuracy, contraindications, duplications, drug‑drug interactions, drug‑food interactions, allergies, and appropriateness of drug, dose, and frequency.
- Provide education, in services, and training to other staff and pharmacy students as indicated.
- Other duties as assigned; hours scheduled and assignments may change to meet departmental needs.
- Provide positive and professional representation of the organization.
- Promote a culture of safety for patients and employees through proper identification, reporting, documentation, and prevention.
- Maintain competency and knowledge of current standards of practice, trends, and developments in related scope of role or practice.
- Maintain compliance with organization’s policies and established practices, protocols, and procedures.
- Complete with organizational and regulatory policies, as well as established practice protocols, of the position, department, and applicable professional standards.
- Handle confidential patient information in accordance with organizational and regulatory policies.
- Demonstrate excellent customer service through attitude and actions consistent with the organization’s Vision, Mission, and Values.
- Adhere to professional standards, hospital policies and procedures, federal, state, and local requirements, and TJC or other accrediting body standards.
Requires critical thinking skills, decisive judgment, and ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action.
Education/Certification- Bachelor of Science in Pharmacy OR Doctor of Pharmacy from an accredited college of pharmacy (required)
- Current NC pharmacy license (required)
- Completion of accredited PGY1 residency program or 3-5 years of equivalent clinical experience (required)
- BCPS and Antimicrobial Stewardship certification (preferred)
- BLS and ACLS required within 120 days of start date (required)
