Job Overview

Fairview Range is looking for a Physical Therapist to join our Hibbing Home Care and Hospice team.

In this role you will coordinate the delivery of physical therapy services appropriate to the needs of the clients of all ages. Develops a plan of care which includes the provision of necessary education and support to the clients and caregivers, monitoring progress, providing care and managing resources. Provides supervision to paraprofessional staff. Collaborates with the Supervisor/Manager on issues.

Schedule

This position will work up to 40 hours per two weeks. The shift length will be 8-hours on the day shifts, with no weekends.

As a Physical Therapist, you will be responsible for:

  • Ongoing development and implementation of the plan of care
  • Maintains an individualized plan of care, related to the client's needs, in collaboration with the physician, client and caregiver. Determines appropriate utilization of resources based on clinical assessment, HHRG guideline and standards of care.
  • Continually reassess the client's health status, environment, and support system with ongoing comprehensive assessment. Plans maximize independence of client/caregiver. Evaluates the client's progress and response to care/services delivered and adjusts the plan accordingly. Continually evaluates whether the client's:
    1) clinical needs can be met at home
    2) are able to care for themselves or have a paid or voluntary primary caregiver
    3) home environment supports home care services.
  • Continually reassess resource utilization as client progresses towards goals. Reviews, prioritizes, and implements POC based on payer limitations.
  • Coordination of services
  • Accountable for managing the clinical episode of care to achieve desired client outcomes and measurable functional goals.
  • Communicates consistently and effectively with client, caregiver, physicians, and all team members to assure appropriate implementation and completion of the client plan of care.
  • Manages client expectations.
  • Advances the POC with the multidisciplinary team. Consults with the other team members as needed to make modifications in the plan. Decreases utilization of services as client moves towards goal achievement.
  • Supervises paraprofessional staff in client care.
  • Professional Intervention
  • Provides safe, effective skilled and advanced intervention, within scope of practice according to client need.
  • Provides counseling and teaching of client, family and caregiver regarding care needs and health promotion/prevention.
  • Evaluates and documents response to care/services delivered.
  • Demonstrates ability to adjust approaches to care to reflect developmental level and cultural differences of populations served.
Required Qualifications
  • Doctorate degree in Physical Therapy
  • 1 year of acute care, long term care or clinic experience
  • Current MN Physical Therapist licensure
Preferred Qualifications
  • 1 year experience in home care
  • Basic Life Support (American Heart Assoc or Red Cross)
