Community Health Worker
Location: Hibbing, MN
OverviewThe Community Health Worker (CHW) is responsible for helping Behavioral Health Home patients and their families access and navigate community services and resources and adopt healthy behaviors. The CHW works with the BHH Social Worker through a coordinated approach to care management and community outreach. Activity will promote, maintain and improve individual and community health identified in the patient treatment plan.
The CHW provides social support and informal counseling, individual health education and advocacy for individuals and community health needs. Providing home visits and in‑clinic visits as determined by the CHW and the BHH team is required.
This position will work 72-80 hours per two weeks. The shift length will be 8 hours per shift.
Weekends:
As needed.
- Promoting wellness by providing culturally appropriate health information to Behavioral Health Home patients.
- Providing individual social and health care support.
- Growing access to primary care through culturally proficient outreach and enrollment strategies.
- Making referrals and coordinating services.
- Advocating for individual and community needs.
- Bridging the gap between communities and the health and social service systems.
- Community Health Worker Certificate through an accredited program or an A.A./A.S. in Health, Human Services, or a related field.
- 3 years' experience working on community health projects, programs, or initiatives.
- Community‑based leadership experience.
- Demonstrated competency and sensitivity in working with diverse populations.
- Basic Life Support (American Heart Association or Red Cross).
- B.S./B.A. in Health, Human Services, or a related field.
- Experience teaching or presenting information to groups.
- Experience with Microsoft Office.
- Multicultural healthcare experience or experience in other countries.
$18.31–$26.37 per hour.
