We are offering up to a $21,000 sign‑on bonus for a three‑year commitment.
Fairview Range is hiring a Physical Therapist for our Home Care and Hospice team in Hibbing, MN. We take great pride in providing compassionate care to our patients and their families while serving communities throughout the expansive Iron Range.
As a dedicated Physical Therapist, you will orchestrate the delivery of personalized physical therapy services, develop comprehensive care plans, provide education and support for clients and caregivers, monitor progress, deliver hands‑on care, manage resources, supervise paraprofessional staff, and collaborate with the Supervisor/Manager.
ScheduleThis position will work 40 to 71 hours per two weeks. The shift length will be 8 hours on the day shift with no weekends.

- Care team includes RNs, Physical Therapists, Occupational Therapists, Social Workers, LPNs, HHAs, Chaplain, Volunteers, Physician Medical Director and a Nurse Practitioner.

As a Physical Therapist you will be responsible for- Planning:
Ongoing development and implementation of the plan of care. - Organizing/Staffing/Directing:
Coordination of services. - Controlling and maintaining complete, accurate and timely clinical records/documentation on all clients according to agency policy.
- Professional intervention.
- Professional development.
- Current MN Physical Therapist licensure.
- One year of acute care, long‑term care, home care or clinic experience.
- Doctorate of Physical Therapy.
- One year experience in home care.
- BLS certification.
Hourly Rate: $36.38–$51.33
