Job Overview
Fairview Range is hiring a Community Health Worker for our Behavioral Health Home Department in Hibbing, MN. The Community Health Worker (CHW) is responsible for helping Behavioral Health Home patients and their families access and navigate community services and resources, adopting healthy behaviors. The CHW works with the BHH Social worker through a coordinated approach to care management and community outreach.
Activity will promote, maintain, and improve individual and community health identified in the patient treatment plan. The CHW provides social support and informal counseling, individual health education, and advocacy for individuals and community health needs. The CHW provides home visits and in‑clinic visits as determined by the CHW and BHH team.
This position will work 72–80 hours per two weeks. Shift length will be 8 hours per shift. Weekends as needed.

Responsibilities- Promote wellness by providing culturally appropriate health information to Behavioral Health Home patients.
- Provide individual social and health care support.
- Grow access to primary care through culturally proficient outreach and enrollment strategies.
- Make referrals and coordinate services.
- Advocate for individual and community needs.
- Bridge the gap between communities and the health and social service systems.
- Community Health Worker Certificate through an accredited program or A.A./A.S. in Health, Human Services, or related field.
- 3 years' experience working on community health projects, programs, or initiatives.
- Community‑based leadership experience.
- Demonstrated competency and sensitivity in working with diverse populations.
- Basic Life Support (American Heart Association or Red Cross).
- B.S./B.A. in Health, Human Services, or related field.
- Experience teaching or presenting information to groups.
- Experience with Microsoft Office.
- Multicultural healthcare experience or healthcare experience in other countries.
