SUMMARY OF POSITION

The Heart Transplant Nurse Coordinator plays a crucial role in the organ transplant process by coordinating patient care and education throughout the pre‑transplant (referral, evaluation, and waitlist management), transplant, and post‑transplant phases of care. The transplant coordinator will maintain regulatory integrity and adherence to established regulations throughout all processes. They act as a central point of contact for patients, families, and the transplant team, ensuring smooth and effective communication and care.

The transplant coordinator will also support the inpatient and outpatient transplant populations. The Transplant Coordinator is an integral member of a larger interdisciplinary team contributing to excellence in patient care, research, teaching, and in providing leadership to the organization.

MINIMUM QUALIFICATION(S)
  • Bachelor's degree in nursing or master's degree in nursing is required.
  • Minimum two (2) years of experience working with cardiac patients and/or two (2) years of critical care experience.
  • Currently licensed to practice as a Registered Nurse by state of employment or holds a multistate RN license through the interstate Nurse Licensure Compact required.
  • AHA BLS prior to end of orientation period
PREFERRED QUALIFICATION(S)
  • Experience working with cardiac transplant and/or mechanical circulatory support patients
  • ACLS preferred
  • Certification as a Clinical Transplant Coordinator (CCTC):
    If Certification is not held at time of hire, must establish a plan with Program Manager for obtaining certification within 3 years of date of hire
