Community Interactions, Inc. is a non‑profit organization dedicated to providing services for more than 200 individuals with intellectual, physical, and behavioral health disabilities.
In general, the work of the LPN is a professional medical position supporting individuals with disabilities receiving services through Community Interactions, Inc. The responsibilities of this position include assisting the individuals in achieving their desired outcomes as identified in their annual plan, performing general office duties, requiring the ability to sit and/or stand for long periods of time. Work outside of the office setting is also expected, including visits to homes and other work sites, and may include lifting and other physical activity.
Travel to program sites and meetings is required.
- Site visits to check medication administration accuracy.
- Site/individual based trainings on diagnoses, meds, and procedures.
- General training for all staff on various health‑related topics.
- Ensure that all pertinent data from past medical history is recorded in the current lifetime medical history.
- Capture all pertinent dates from medical appointment documentation throughout the year and annually update lifetime medical history at the time of the annual planning meeting.
- Establish contact with hospital when a consumer is hospitalized and assume primary point of contact.
- Coordinate discharge plans with hospital staff.
- Ensure that adequate supports are in place for the individual once hospital discharge has occurred.
- Attempt to obtain pertinent hospital records at the time of discharge.
- Assume primary point of contact for pharmacies regarding medication issues.
- Attend planning meetings to provide any necessary medical information.
- Ensure that the individuals’ healthcare needs are considered in all aspects of planning.
- Advocate for the individual’s health care needs.
- Maintain relationships with members of the individual’s team to sustain lines of communication in the event of any health‑related issues that need to be addressed.
- On‑call responsibilities for assessment of medical issues as needed; review hospital discharge instructions and insure that changes are completed to assure the health and safety of the individuals.
All other duties may be required by the Director of Incident Management and Quality Assurance.
QualificationsMust hold a Practical Nurse license in the state of Pennsylvania. Three years minimum experience in the nursing field. Experience working with individuals with disabilities is preferred.
