Breast Health Navigator - Full Time Days at Williamson Health summary:

The Breast Health Navigator is a licensed nurse who coordinates and implements care for patients with breast health concerns, providing clinical navigation, education, outreach, and barrier removal across inpatient and outpatient settings. Responsibilities include initiating patient contact, organizing appointments, guiding patients and families through treatment plans, documenting interactions and outcomes, and connecting patients to community resources and multidisciplinary teams. The role requires nursing licensure, a nursing degree, attainment of Breast Health Navigation certification within 18 months, and the ability to work clinically, travel for outreach, and support program metrics and conferences.

Position Summary
To coordinate, implement, and plan nursing care to persons with breast health concerns, including the promotion of breast health in a timely and accurate manner.

Position Requirements
Formal Education / Training:
1. Licensed by the State of Tennessee.
2. Earned a nursing diploma or degree form a college or university
3. Breast Health Navigation Certification (credentialed) within 18 months of employment

Workplace Experience:
Coordination of Patient Care Delivery for Breast Health Services

Equipment and Skills Training:
Standard patient care equipment for services provided. The ability to work well under pressure, to function dependently and independently and to be flexible in applying basic nursing knowledge to a variety of setting. Equipment:
Network Computer System

Physical Environment:
An Oncology medical/surgical unit and outpatient clinic whose patient population ranges consists of adults > 18yrs of age.

Physical Effort:
1. Able to communicate in English verbally and legibly.
2. Able to stand/work for up to eight hours a day.
3. Ability to travel when needed to area Breast Cancer functions.
3. Able to lift with assistance up to 50 lbs.

KEY RESULTS Performance Standards (Weight = 50%)
1. Plans, develops, and oversees the day-to-day clinical navigation activities for Breast Health Patients.
2. Initiates contact with breast patients and provides support to the patient in order to navigate the healthcare system.
