Community Support Worker

This is a part-time remote role for a Community Support Worker. The Community Support Worker will be responsible for assisting individuals with daily living activities, providing support to meet their physical and emotional needs, and helping clients integrate and thrive within their local communities. The role involves engaging in effective communication and collaborating with case managers to ensure personalized and high-quality care for clients.

• Strong interpersonal skills and the ability to build supportive and positive relationships
• Experience and skills in Community Support and Community Health
• Effective communication skills to collaborate with clients, case managers, and other stakeholders
• Knowledge of or experience in case management within a healthcare or community support setting
• Dedication to providing compassionate, client-focused support
• Ability to work independently and remotely while maintaining close collaboration with the team
• Previous experience in a similar role is an advantage
• A relevant certification or qualification in health and social care is preferred
