Occupational Therapist
Location: Plough Lane, Hereford, HR4 0LE

Start Date: ASAP
Contract Duration: Permanent
Working Hours: Mon Fri, 09 hours per week
Pay Rate: £36,363.00 per year

Job Responsibilities
The successful candidate will:

  • Work in partnership with adults, their families, and care givers to address restrictions caused by illness or medical conditions.

  • Conduct functional, social, and environmental needs assessments through observation, therapeutic, and direct engagement.

  • Identify risks and barriers to independence and establish individuals' strengths, motivations, and independent capabilities.

  • Create meaningful, personalized programs to qualify access to available services and support.

  • Work autonomously in either fixed, field, or flexible roles using a holistic, person-centered approach.

  • Focus on early intervention to reduce, delay, or prevent the escalation of health, care, and support needs.

  • Provide bespoke interventions, information, and advice in a person's home and local environment.

  • Facilitate the provision of aids to daily living, adaptations, dignity with moving & handling, specialist community equipment, and technology-enabled living.

  • Maintain a varied caseload, undertake statutory assessments, and deliver and evaluate a range of supportive intervention solutions.

  • Support the supervision of junior staff.

Person Specifications
Must Have

  • Registration as a qualified Occupational Therapist with the HCPC.

  • Strong interpersonal, communication, and organizational skills.

  • Commitment to ongoing professional, service, and personal development.

  • Ability to contribute to team building and continuous service improvement.

Nice to Have

  • Experience working with a diverse service user group in various rural, town, village, and town settings.

  • Fieldwork experience that can be developed into professional, management, and supervisory skills.
