Registered Nurse (RN) Home Health - Full Time / Part Time (Per Diem)

We are a trusted provider of home healthcare and hospice services. We are looking for a compassionate, organized, and team-oriented RN to join our Home Health team as a Full‑Time or Part‑Time (Per Diem) RN.

As a Registered Nurse, you will evaluate patients, create and implement care plans, and track progress to maintain the highest standards of care. You’ll work closely with our Clinical Director and staff, providing support both remotely and independently in the field.

• Provide nursing care within the scope of practice following the patient’s individualized care plan in coordination with the interdisciplinary team.
• Administer treatments, monitor patient conditions, and report changes to Physician, Case Manager, or Director of Nursing as needed.
• Maintain accurate clinical records and submit documentation using a company‑provided device.
• Supervise LVNs when assigned.
• Comply with all company policies and procedures.

• Current RN license to practice in California.
• At least 1 year experience in home health or hospice care.
• Valid CA driver’s license.
• Able to travel within a 15–25 mile radius regularly.

$50–$70 per hour (depending on experience).
