Position: Intake Coordinator / LVN

Overview

Blize Healthcare is seeking a detail-oriented and compassionate Intake Coordinator / Licensed Vocational Nurse to join our team. This role is essential in ensuring patients experience a smooth transition into home health and hospice care. You will support the intake process, ensuring patients and families receive the guidance and coordination needed for a smooth admission. Also, you will provide skilled nursing care to patients in their homes, often under the direction of a Registered Nurse or physician.

- Job Requirements
  • Completing insurance forms and answering patient inquiries in a timely and professional manner.
  • Scheduling home health and hospice visits efficiently.
  • Handling daily patient referral and intake operations.
  • Verifying insurance coverage and assisting the billing department with processing claims.
  • Ensuring a smooth transition of patients into home health or hospice care.
  • Answering, screening, and directing phone calls.
  • Taking messages and schedule appointments.
  • Maintaining records of medical supply inventory.
  • Selecting and packaging DME (Durable Medical Equipment) supplies.
Licensed Vocational Nurse (LVN)

Qualifications:

Skills and Qualifications
  • Minimum 1 year of experience in the medical field and minimum 1 year of nursing experience.
  • Home health or hospice experience strongly preferred.
  • Proficient in basic nursing skills (medication administration, wound care, and vital sign measurement).
  • Excellent verbal communication, interpersonal, and customer service skills.
  • Compassionate with the ability to connect with patients and families on an emotional level.
  • Strong problem-solving skills with the ability to identify and resolve patient concerns in a timely manner.
  • Basic understanding of administrative and clerical procedures and systems.
  • Proficient with Microsoft Office (Word, Excel, Outlook) and Google Suite (Docs, Sheets).
License/Certification
  • LVN license.
  • Valid BLS certification.
  • Driver's License.
