Under the direct supervision of the Director of Rehab Services the Rehab Clinical Coordinator provides high quality, patient-centered care by managing the complete process of therapeutic care of an assigned group of patients, in coordination with the physician and the patient care team. The Coordinator assists with staffing schedules and ensures that all area assignments are covered daily and covers vacancies as needed.
The Coordinator will complete special projects, as directed by their superior. The Coordinator will provide feedback to the Director of Rehab Services and the VP Operations in employee relations matters, including any disciplinary action. The Coordinator will monitor all supply needs. In addition to their responsibilities, this individual will also take part in patient assessments, and participates in developing and implementing individualized care plans, focusing on goal achievement, patient engagement, and positive patient experience.
The Coordinator provides or directs a majority of the patient’s therapy to support patients throughout the continuum of care.
What you will do in this role:
- Providers evaluations and treatment interventions within the scope of practice established by law and professional code of ethics.
- Assesses and evaluates therapeutic/rehabilitative/functional states, and participates in the development of the patient’s plan of care.
- Provides purposeful and skilled therapeutic care, using procedures and techniques to produce a change in condition/function consistent with the physician’s plan of care.
- Observes and reports patient’s reaction to treatment and changes in patient’s condition.
- Instructs patient/family/caregiver in the use of interventions.
- Ensures accurate, complete, and timely clinical documentation in accordance with services/departmental guidelines.
- Communicates pertinent payor needs/requirements to physician, treatment team, patient and patient’s family in a timely manner.
- Prioritizes caseload and, as appropriate, determines daily staffing needs and coordinates assignments/handoffs, Demonstrates efficient and appropriate use of support staff.
