TLC Health & Rehab is seeking a Full-Time Admissions Nurse to join our team!
The Admissions Nurse plays a key role in welcoming new residents and ensuring a smooth and efficient admission process. This position works closely with the clinical team, admissions department, and families to coordinate resident admissions and provide excellent customer service.
Position: Admissions Nurse
Status: Full-Time
Location: TLC Health & Rehab
Responsibilities- Coordinating and completing resident admissions
- Reviewing hospital referrals and clinical documentation
- Communicating with hospitals, families, and internal teams
- Completing admission assessments and required paperwork
- Ensuring a positive admission experience for residents and families
- Active RN or LPN license
- Skilled Nursing Facility (SNF) experience preferred
- Strong clinical assessment and communication skills
- Ability to work collaboratively with interdisciplinary teams
