Job Summary:
This post is for a STaR worker for our Flexible Assertive Community Treatment Team (FACT). The FACT Team sits within the Adult Community Mental Health Service (ACMHS) and provides short‑term assertive interventions to service users under the ACMHS. This will typically be service users who need support with engagement or need a brief period of increased support.
You will work under the supervision of a dedicated FACT clinical lead, as part of a team which provides mental health services. This post covers all of the North West of Hertfordshire, including St. Albans, Harpenden, Hemel Hempstead, Tring, Berkhamsted and the surrounding area; you will need to be able to travel between these places. You will work within the Care Programme Approach / Care Management process with a focus on Recovery Principles and provide support, give time and thus promote recovery.
Mainduties of the job
- Complete regular visits within the remit of the agreed care plan.
- Home visit/interventions offered to all those within the catchment area.
- Connected Lives Assessments.
- Manage medication with support of the FACT lead or nurse.
- Liaise with GP's and Carers/service providers where felt appropriate.
- Providing evidence‑based interventions.
- Work as part of the wider team.
- Provide MDT input where appropriate.
- Support Service user to complete the Dialog.
- Risk assess as part of the interventions offered.
- To provide support, give time to an allocated group of service users and promote their recovery and promote their place in the broader community.
- To ensure that service users engage beneficially with the agreed Care Plan and access appropriate services provided on a regular and consistent basis.
- To positively promote independent living of service users within community.

Person Specification Qualifications- NVQ level 3 qualification or mental health certificate level 3 or agreement to undertake such training as soon as resources permit
- Commitment to Continuing Personal Development
- Three years experience of providing mental health services from within a team setting, which may be combined with personal lived experience of mental health difficulties
