Memory Nurse

An exciting opportunity for a Band 6 Memory Nurse to join our Countywide EMDASS Team delivering service for West Hertfordshire. The team is multi‑disciplinary and includes Team Secretaries, Dementia Nurse Specialists, Deputy Community Manager, Service Manager, Psychologists, Psychology Assistant, Occupational Therapists, Psychiatrists, GP and partner agency staff. Excellent organisational and communication skills alongside solid clinical experience are essential for this post.

The post‑holder will undertake memory assessments for service users suspected of dementia, deliver diagnosis in collaboration with the multi‑disciplinary team, and provide treatment and post‑diagnostic support through titration reviews. Work will span the One‑Stop Pathway and Primary Care.

The role includes carrying out risk assessments, implementing risk management plans, safeguarding, and making referrals to community services. The post‑holder will act as Duty Nurse for screening referrals, offering advice and signposting, and supervise junior staff and trainees as delegated by the MATS Team Leader.

Job responsibilities

The post‑holder will carry out memory assessments using appropriate cognitive tools, work with the multi‑disciplinary team to deliver memory diagnosis, and develop specialist care plans in line with NICE guidelines. Responsibilities also include:

  • Developing care plans that promote independent community living
  • Completing risk assessments and clear management plans for identified risks
  • Providing ongoing post‑diagnostic support where indicated
  • Discharging with appropriate care provisions and arranging future reviews
  • Maintaining a high standard of record keeping and documentation in line with Trust policies
  • Co‑operating with colleagues to promote the services philosophy, framework and strategic objectives
  • Contributing to dementia strategy discussions and steering groups when requested by senior management
  • Demonstrating clinical expertise, high standards of practice, and person‑centred care reflecting best practice
  • Applying legal and ethical knowledge of dementia care, including the National Dementia Strategy
  • Completing Revalidation as defined by the NMC
  • Managing, supervising, training and developing students/trainees or equivalent others and conducting yearly appraisals
  • Planning and organising diaries, prioritising duties and coordinating the care of allocated service users, with due regard for health and safety
  • Promoting staff personal health and wellbeing and accessing help when needed
  • Reporting potential fraud and completing duty and expense claims, ensuring all EMDASS policies are implemented
  • Participating in service transformation, making proposals for pathway improvement, and conducting audits where accepted
  • Communicating clinical information verbally and in writing to patients, families, carers and professional colleagues
  • Ensuring patient confidentiality and adhering to documentation standards
  • Negotiating and problem‑solving in contentious situations and maintaining working relationships with all agencies
  • Applying the Mental Health Act, Mental Capacity Act, National Dementia Strategy, National Service Frameworks, and Safeguarding Adults procedures in decision making
  • Prioritising and organising casework daily, ensuring high‑quality care, seeking assistance when required, and representing EMDASS at meetings as delegated
  • Driving to meetings, training, and occasionally covering for colleagues on other sites, accepting travel to out‑of‑county homes for assessments
  • Participating in RESPECT and Basic Life Support trainings as required by HPFT policies
