Social Worker – Community Care Team
Location: Helensburgh, UK
Pay Rate: £43 per hour
Contract Type: Locum, Ongoing
Hours: 35 hours per week

What you will do:

Undertake person-centred assessments, develop care plans, and complete reviews for adult service users

Work collaboratively with partner agencies and professionals to deliver a holistic approach to community care

Provide advice, support, and guidance to families and carers, signposting to appropriate services

Maintain accurate, timely, and comprehensive records and reports in line with departmental requirements

Travel across Helensburgh and Lochside to visit service users and attend meetings as required
