, you’ll jump in where needed, supporting patients and families through their care journey while partnering with multidisciplinary teams to keep everything running smoothly.

• Coordinate care from admission through discharge
• Advocate for patients and families
• Collaborate with treatment teams and community resources
• Keep plans organized, updated, and compliant
• Bring empathy, organization, and problem-solving to every shift

This position starts at $23.65 per hour.

• Required education and experience - Combination of Bachelor’s degree in a human services field with at least 1 year of full-time experience serving people with mental illness.
• Preferred education and experience - 2 years of experience in case management or patient care in a healthcare or social services delivery system preferred. Experience in an inpatient care setting is also preferred.
