We are looking for highly skilled,enthusiastic, dynamic self-motivated leader to lead our Frailty Team based at Clarendon, Hebburn and Community Falls Team based at Washington Galleries.
Would you enjoy the challenge of providing senior support and clinical leadership in the delivery of high-quality care forthe patients in the community setting.

You will be responsible for the leadership and management of the Frailty Team consisting of Band 7 specialistnurses and Community Falls Team consisting of Band 6 nurses, physiotherapistsand HCAs. You will be responsible for the day-to-day Leadership and Management of the Team and will be responsible for governance and assurance.

The post holder will be accountable for the overall management and development of the team and supervision of staff and ensuring the coordination of services provided,optimising the cost-effective use of all resources.
Line management of the team, withoverall accountability and responsibility for the delivery of all aspects ofpatient / client care through the provision of effective clinical andmanagerial leadership.
To lead, motivate and support theteam approach to delivering new and innovative models of care for patients /clients.
To lead a continuously improvingservice, by ensuring systems are in place to support clinical governance,quality monitoring and ongoing research and audit of practice.

- To work autonomously, assessing individual patient / client needs, initiatinginvestigations, determining a plan of care and initiating appropriate holistic,research based health information and care.
- To act asa resource for all members of the Multi-Disciplinary Team, providing expert advice concerning aspects of patient / client management.
- To be responsible for ensuring that agreed professional standards are reflected in practice and to have overall responsibility for monitoring the standard ofcare.
