We are seeking an experienced, self‑driven Full‑Time or Part‑Time (Per Diem)
Registered Nurse (RN) to join our dedicated Home Health team and provide exceptional care throughout the region.

• Provide nursing care within scope of practice, following individualized care plans, in coordination with the interdisciplinary team.
• Administer treatments, monitor patient conditions, and report signs/symptoms indicating a change in condition to Physician, Case Manager, or Director of Nursing.
• Maintain accurate clinical records and submit documentation per company policy using a company‑provided device.
• Supervise Licensed Vocational Nurses (LVNs) when assigned.
• Comply with all company policies and procedures to ensure high standards of care.

• Current Registered Nurse (RN) License to practice in California – Required.
• Experience in Home Health or Hospice care – 1 year required.
• Current California Driver’s License.
• Ability to travel within a 15–25 mile radius regularly.
