If yes, how you will contribute to the team:
Under the direction of the Social Services Director, our Social Services Designee will assist each resident and family in adjusting to placement, illness, and the plan of care, helping them attain the highest practicable level of functioning in accordance with current federal and state regulations, and our policies and procedures. You will assist residents and families with wishes regarding treatment and help with their transition and adjustment to the facility.
You will also participate in resident and family council meetings as requested, and support Admissions and assist with discharge planning services.
Must Have:
- High school diploma or equivalent.
- Ability to work with physically and mentally ill, disabled, and emotionally upset elderly individuals.
- Ability to organize, prioritize, and meet deadlines.
- Ability to maintain confidentiality and be discreet.
- Knowledge of nursing practices, procedures, terminology, laws, regulations, and guidelines that pertain to long-term care.
Nice to Have:
- Experience with geriatric population.
- AA or Bachelor's degree in a related field.
- Creative problem-solving skills.
- Sufficient computer skills to manage records and documentation.
