Nurse Practitioner - Primary Care Hays CBOC

Summary

This position is eligible for the Education Debt Reduction Program (EDRP) – a student loan payment reimbursement program. Applicants must meet specific eligibility requirements per VHA policy and submit an EDRP application within four months of appointment. Program approval, award amount (up to $200,000) and eligibility period (1–5 years) are determined by the VHA Education Loan Repayment Services program office after review of the application.

• The Robert J Dole VA Medical Center (VAMC) is seeking a full‑time Advanced Practice Nurse (APN), Certified Nurse Practitioner (NP) responsible for providing Primary Care services to Veterans of the Robert J Dole VAMC in Hays, KS.
• The NP practices in Primary Care Services, providing a full range of services emphasizing quality, timeliness, and the advancement of the Veterans Affairs (VA) mission.
• The NP provides advanced clinical practice, consultation, and management.
• The NP is a licensed independent practitioner with advanced education and national certification who provides evidence‑based care for a patient population.
• Independently manages a panel of patients with acute and chronic health conditions, focusing on health promotion and disease prevention using evidence‑based practice.
• Develops and implements an individualized plan to achieve optimal population health and disease management.
• Demonstrates leadership and works collaboratively with the Primary Care Team and other interdisciplinary team members.
• Orders and interprets labs, tests, and diagnostic studies.
• Prescribes pharmacologic and non‑pharmacologic interventions.
• Initiates referrals and consults to specialty services.
• Provides education to patients and their families.
• Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes.
