Overview
At the direction of the Clinical Manager, coordinates/manages a case load comprised of Hospice patients. Assesses, plans, implements, coordinates, and evaluates care for each patient/family in conjunction with the hospice interdisciplinary team. Case management includes but is not limited to comprehensive needs assessments, development of appropriate plan of care, referral to internal and external resources, evaluation of outcomes, and provision of nursing educational, clinical and related services.
Supervises the Licensed Nursing Assistants/ Home Health Aides.

Key Performance Indicators- Timeliness of clinician documentation (within 24hours)
- Meets productivity standards
- Maintains compliance with HOPE visits
Required Qualifications
- Must be a licensed Registered Nurse in Vermont and New Hampshire or hold a compact license.
- Graduate of an accredited school of nursing
- Minimum two years’ general experience in a supervised nursing setting recommended.
- Hospice/Homecare experience preferred.
- Certified in Hospice/Palliative Care Nursing desirable.
Licensure/Certification s
– Unencumbered RN license in VT and NH or compact.

Nursing - Pay Range: $85,280.00/Yr. – $/Yr. (Based on 40 hours per week, otherwise pro rata)
- FTE/Hours per pay period: 1.00 – 1.00 – 40 hrs/week
