Position Overview
Physical Therapists at Visiting Nurse and Hospice for Vermont & New Hampshire (VNH) screen, evaluate, re-evaluate, and treat patients in home health settings. They coordinate and communicate treatment activities with an interdisciplinary team and the patient, ensuring quality care and optimal outcomes.

• Evaluate the patient's need for physical therapy and develop an appropriate treatment plan.
• Carry out appropriate treatment programs in accordance with agency policies and professional standards of care.
• Communicate with all members of the health care team.
• Document evaluations, treatment goals, and objectives.
• Teach patients and family members the rationale for treatments and procedures.

• BS in Physical Therapy
• Physical Therapist license in both VT and NH or possession of a compact license
• One year of experience as a PT in a home health care setting

• Minimum: $39.25/hr
• Maximum: $60.84/hr
