  Lebanon, NH area;
  Bradford, VT area;
  Windsor, VT area.
• Play a crucial role in providing superior nursing, rehabilitation, hospice, and personal care services to over 70 towns in the community.
• Evaluating the patient's need for physical therapy and developing personalized treatment plans.
• Implementing appropriate treatment programs in line with professional standards.
• Communicating effectively with all members of the healthcare team.
• Documenting evaluations, treatment goals, and objectives clearly.
• Educating patients and their families about treatments and procedures.

• BS in Physical Therapy.
• Licensed Physical Therapist in VT and NH or hold an unencumbered compact license.
• One year of experience as a PT in a home healthcare setting.

• BLS/BCLS.
• ASHACHTACP Training.
