As a home health Physical Therapist in our Rutland office, servicing Rutland and surrounding areas, you will be an integral member of a multi-disciplinary health care team that provides rehabilitative care and skilled nursing to clients, affording them the opportunity to receive the medical care required to remain  believe our clients and their families deserve the highest quality home health care delivered with compassion, excellence, and reliability.

• Base Pay: $63 - $70 / point depending on qualifications
• Flexible scheduling (full-time, part-time, various shifts)

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements (mobility, range of joints, transfer status, stability, ambulation patterns, strength, endurance, balance, safety).
• Develop and implement individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
• Continually assess and revise the Physical Therapy care plan.
• Educate and instruct clients, family members, or other representatives in rehabilitative care and activities to promote health, safety, and independent living.
• Accurately document observations, interventions, and evaluations pertaining to client care management.

• A current Physical Therapist (PT) license in the state
• A minimum of one year of recent work experience as a Physical Therapist
• Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association, as indicated by school transcript or diploma
• Solid computer skills; prior experience with electronic medical records (EMR) preferred
