The State of Connecticut, Department of Social Services (DSS) – Division of Health Services – is accepting applications for two (2) Developmental Services Case Manager positions located in Hartford, CT.

The Developmental Services Case Manager is responsible for coordinating, monitoring, and supporting services for individuals receiving developmental disability services. This position ensures compliance with federal waiver requirements, promotes continuity of care, and supports individuals and families in accessing and maintaining needed services and benefits. The Case Manager works collaboratively with individuals, families, providers, and interdisciplinary teams to develop, implement, and monitor Individual Service Plans (ISPs).

This Case Management position is specifically in the Autism Waiver Services Division in the DSS Community Options Unit.
