Shift Detail: nonclinical APP to make 3-5 days post-discharge calls to complex patients, ensure medication reconciliation scripts are filled and üret

Position Details: Hartford Hospital seeks a Physician Assistant or Nurse Practitioner to join the Department of Medicine team. The role involves non-clinical outreach post-discharge calls, medication reconciliation, and follow-up coordination with PCPs and specialists.

Role: Open to Physician Assistants or Acute Care Nurse Practitioners

Setting: Inpatient
Schedule: Per diem (20‑24 hours weekly)

  • Use Epic AI model to identify high-risk patients at discharge
  • Reduce readmission rates for high-risk, high VVI score patients
  • Target patients with ≥22% predicted risk of readmission and discharged to home

Qualifications:

  • Licensure:
    Unrestricted State of Connecticut Medical License (or eligible)
  • Training:
    Certified or board-eligible Physician Assistant or Acute Care Nurse Practitioner
  • Experience:

    Minimum of 5 years required
