APP Cardiology - Outpatient

Cardiology Outpatient Advanced Practitioner opportunity in the Hartford CT region.

Trinity Health Of New England seeks aNurse Practitioner or Physician Assistantto join the Cardiology Heart Failureteam at Saint Francis Hospital and Medical Center.

RESPONSIBILITIES:

• See outpatients in various Heart Failure Centers throughout the region including St. Francis Hospital and Medical Center in Hartford. One site per day. Training begins at SFHMC.
• Provide transitional care management
• Offer guideline directed medical therapy optimization
• Perform advanced heart failure evaluations
• Provide same day access care

The successful candidate will:

• Interested in heart failure care
• Have Cardiology/Heart Failure experience (preferred but not required)
• Hold an Advanced Practice Nurse or Physician Assistant CT state license.
