Affirmed Home Care is expanding to serve clients in the Hartford, CT region, and we are seeking dedicated Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) to provide exceptional, one‑on‑one care in the comfort of our clients’ homes.

• Competitive Compensation (LPN): $38–$45 per hour, based on license type and experience (LPN's)
• Competitive Compensation (RN): $55–$65 per hour, based on license type and experience (RN's)

• Conduct thorough client assessments and implement individualized care plans.
• Administer medications, treatments, and nursing procedures in accordance with physician orders.
• Monitor, evaluate, and document client conditions, communicating updates promptly to care teams.
• Deliver advanced nursing interventions, including wound care, IV therapy, and catheter management (RNs).
• Educate clients and families about health maintenance, medication compliance, and home safety.
• Maintain accurate, detailed documentation following regulatory and agency standards.
• Ensure compliance with all state, federal, and company healthcare protocols.
• Provide compassionate, patient‑centered care that reflects Affimhome Home Care’s values of dignity and excellence.

• Active RN or LPN License in the State of Connecticut (required)
• Minimum 1–2 years of nursing experience (home care, geriatric, or related field preferred)
• Strong assessment and clinical decision‑making skills
• Excellent communication, time management, and interpersonal abilities
• CPR/BLS certification (required)
• Reliable transportation and valid driver’s license
• Experience with Electronic Health Records (EHR) preferred
