Position

Purpose:

Provides skilled nursing care to the organization's clients in accordance with the plan of care as prescribed by the physician and assigned by the supervising registered nurse.

Position Details:

Schedule:

Monday-Friday, 8 am to 4:30 pm, weekend/holiday rotation as required

Coverage Region:
Waterbury

NOTE:

This position is 100% home care providing services in patients homes.

What you will do:

* Plan and coordinate client visits based on physician orders, client needs, and available resources.

* Administer medications and perform treatments according to care plans.

* Assess client conditions through observation and evaluation; report changes promptly and update care plans as needed.

* Educate clients and families on care procedures and health management.

* Document all care activities accurately and submit reports in a timely manner.

* Collaborate with healthcare team during care conferences and maintain effective communication with clients and families.

* Ensure compliance with federal, state, and agency regulations, policies, and ethical standards.

* Support continuous quality improvement and uphold organizational mission and values.

Minimum Qualifications:

* Graduate from an accredited or state approved school of practical nursing

* Current licensure in the state of practice

* Must have a minimum of one (1) year experience in an acute care setting.

* Home healthcare experience preferred.

* Must possess interpersonal skills sufficient to interact effectively with clients and their families as well as all interdisciplinary caregivers, peers, subordinates and supervisors.

* Must have passed National Association Practical Nurse Exam (NAPNES) or equivalent medication exam before administering medications.

* Must have current Driver's license and reliable transportation to and from work site.

* Ability to consistently demonstrate the Organizational Core Competencies and Organizational
