Infectious Disease - Per Diem - Nurse Practitioner (APRN) or Physician Assistant

Shift: Tues, Thurs (Per Diem)

Job Summary: Functioning within the framework for professional nursing practice, the Community Care Advanced Practice Nurse or Physician Assistant works to enhance patient throughput, prevent care gaps, and improve health outcomes. This role supports Hartford Health Care’s mission to improve health for people and communities. Under provider direction, the Community Care Nurse Navigator provides skilled nursing care across settings and reports to a Practice Manager.

• Act as a member of an interprofessional care team in an expanded nurse practitioner role, helping patients transition from acute care to community, reducing readmissions, and improving coordination.
• Partner with inpatient/ED physicians to identify transitional care gaps, establish safe transition plans, and ensure transportation, DME, medication plans, and diagnostic workups are completed.
• Perform post-discharge care strategies within 24‑48 h, including phone calls, education, symptom management, medication reconciliation, and coordination with CCC clinic to minimize gaps.
• Facilitate diagnostic workups, follow up on unperformed tests, collaborate with homecare, pharmacy, and DME to secure supplies, obtain authorizations, and schedule additional appointments.
• Collaborate with clinic physicians to resolve issues and advance treatment plans until a primary care provider is established.

• Current APRN or Physician Assistant license.
• Certification according to CT state law.
• Specific experience in specialty may be required.
• Open to new graduates and experienced APP providers.
