Community Care Coordinator – Primary Care – Full Time

Join the team at Connecticut Children’s. We are looking for a dedicated Community Care Coordinator to support families and children in the North Central Region of Connecticut.

Job Description

Connecticut Children’s Center for Care Coordination (The Center) is dedicated to the integration of care coordination through the delivery of innovative programs, technical assistance, and best‑practice dissemination. The Center utilizes a universal, evidence‑based, research‑informed, and policy‑driven approach to enhanced care coordination that meets the medical, developmental, behavioral, and social needs of children while strengthening family caregiving capacity.

Responsibilities
  • Provide comprehensive care coordination services to children, youth, and their families by linking them to appropriate medical, developmental, behavioral, educational, and social services and resources.
  • Use the Strengths‑Based Approach, Strengthening Families™ and the 5 Protective Factors Framework to partner with families.
  • In collaboration with families and in consultation with primary care and specialty providers, develop and implement a comprehensive, shared plan of care that addresses specific needs and goals.
  • Collaborate with inpatient, outpatient, and behavioral health team members to ensure smooth transitions across the continuum and to mitigate gaps, redundancy, duplication, and poor health outcomes.
  • Advocate for families during medical appointments, school meetings, case conferences, and other relevant settings.
  • Participate in grant‑specific meetings, care‑coordination collaboratives, and community meetings; meet required grant deliverables.
  • Participate fully in Center activities: staff meetings, work groups, innovative programs, projects, and research activities.
  • Communicate with providers via fax/email regarding progress of services.
  • Document all care‑coordination activities in databases as appropriate and adhere to HIPAA confidentiality guidelines.
  • Empower families to direct the care of their children within the medical home model by formulating an advocacy‑focused care plan.
  • Provide education, consultation, referrals, training, and support to families and community providers through direct advocacy, technical training, and educational materials.
  • Collaborate with Center colleagues by providing information, attending care‑management meetings, and assisting in the development of policies and procedures.
  • Address issues impacting cost and quality of care, liaison with Connecticut Children’s Medical Center on‑site and satellite programs.
  • Demonstrate knowledge of population‑specific differences and needs from neonate through adolescence and apply them to practice.
  • Demonstrate cultural sensitivity in all interactions with patients, families, and co‑workers.
  • Show support for the mission, values, and goals of the organization through consistent behaviors.
Qualifications

Education and/or Experience Required

  • Education:

    Bachelor’s degree in Social Work, Family and Human Development, Public Health, or related fields.
  • Experience:

    Minimum of 3 years working with families and the community system (may substitute for degree).

License and/or Certification Required

  • Valid CT Driver’s License; willingness to drive own vehicle throughout North Central Region and State of CT.

Knowledge, Skills & Abilities

  • Knowledge of community resources, social service organizations, cultural diversity, and children, youth, and families.
  • Skills:

    Working with clinical providers as part of a multidisciplinary team; experience in clinical or community settings; excellent written and verbal skills; computer skills for data entry into program databases.
  • Abilities:
    Work with families of all ethnic and socioeconomic backgrounds; work independently with strong time‑management and organizational skills; provide developmentally appropriate care; work within job boundaries; collaborate collegially with all disciplines; excellent communication; bilingual (Spanish/English) preferred.
