Join Trinity Health Of New England At Home! This contingent position covers shifts Monday‑Friday from 8 am to 5 pm. Applicants must be available for at least 1–2 days per week, with occasional weekend and holiday availability.
Position SummaryHospice RN Case Manager Position – Contingent. Our hospice RNs use cutting‑edge technology and clinical knowledge to provide exceptional, compassionate care to patients with life‑limiting or terminal illnesses in their homes. Responsibilities include patient assessment, symptom control and overall supervision of patient care and outcomes. Nurses provide support and comfort, focusing on helping patients manage pain.

Minimum Qualifications- Graduate of an approved nursing education program
- Licensure as a Registered Nurse in the State of Connecticut
- One (1) year experience as a professional acute care nurse
- Hospice home care experience preferred but not required
- Provide one‑to‑one care with patients in their homes
