Job Description
The Clinical Pharmacist – Hematology/Oncology & BMT Specialist is a licensed pharmacist who serves as the medication‑therapy expert for the Hematology/Oncology and Stem Cell Transplant (BMT) programs. The role includes designing, managing, monitoring, verifying, and dispensing drug therapies—including chemotherapy, cellular therapy, immunotherapy, and transplant‑support medications—in alignment with institutional SOPs, ACLS/PALS (as applicable), USP standards, and regulatory requirements. The specialist works collaboratively across inpatient and outpatient hematology/onscology and BMT services, oversees pharmacy technician and intern staff, ensures compliance with hospital, state, and federal medication‑management standards, and supports hospital‑wide pharmacy operations, including Central Pharmacy coverage as staff or charge pharmacist.

Responsibilities

Clinical Practice & Medication Expertise
  • Optimize medication therapy for hematopoietic cell transplant, CAR‑T, and bone marrow transplant patients, including assessment, design, and adjustment of individualized treatment plans.
  • Participate in daily rounds with ACGT, Oncology Infusion, and Inpatient Hematology/Oncology services and maintain continuous monitoring of assigned patients through chart review and interdisciplinary communication.
  • Provide ongoing medication therapy monitoring by reviewing charts, communicating with the healthcare team and families as appropriate, and identifying or resolving drug‑therapy problems.
  • Develop, implement, and maintain evidence‑based protocols, chemotherapy regimens, and treatment pathways in collaboration with multidisciplinary teams and in alignment with COG, NCCN, and other clinical guidelines.
  • Utilize the facility formulary to promote safe, cost‑effective care and provide patient or caregiver medication counseling as needed.
  • Serve as a subject matter expert in hematopoietic cell transplant pharmacotherapy, including CAR‑T and BMT treatments.
  • Collaborate with the ACGT Medical Director to achieve and maintain FACT accreditation.
Pharmacy Operations
  • Manage daily pharmacy workflow across the ACGT Unit, Oncology Infusion Center, and Inpatient Hematology/Oncology units, while supporting Central Pharmacy operations and cross‑covering hospital departments as needed.
  • Verify and evaluate medication orders for accuracy, completeness, and clinical appropriateness, considering patient‑specific factors, adverse effects, and drug‑–drug/drug‑–disease interactions.
  • Supervise technicians, interns, residents, and pharmacists to ensure safe, accurate preparation, dispensing, and delivery of medications in compliance with federal and Connecticut state regulations and pharmacy SOPs.
  • Serve as Pharmacist‑in‑Charge for Central Pharmacy when required.
  • Ensure adherence to all pharmacy SOPs and medication‑safety standards to mitigate risk to patients and staff.
  • Perform daily activities including answering medication‑related inquiries, attending medical rounds, completing nursing unit inspections, and identifying and resolving medication‑related issues.
Education, Mentorship & Professional Development
  • Mentor and train pharmacists, residents, interns, and technicians in oncology, BMT, and CAR‑T pharmacy practice, including regimen validation, preparation, administration considerations, and clinical monitoring.
  • Provide education to pharmacy and multidisciplinary staff on chemotherapy regimens, cellular therapy processes, and best practices in medication management.
  • Contribute to formulary management for oncology, cellular, and gene therapies, including preparation of P&T monographs, education materials, and clinical evaluations.
Regulatory Documentation & Compliance
  • Document clinical interventions, medication monitoring, adverse drug events, and therapy outcomes in the electronic health record (such as Epic, Dose Edge, or Omnicell) in accordance with state and federal requirements.
  • Maintain compliance with Connecticut drug control regulations and all federal requirements governing investigational and non‑investigational medications, including storage, preparation, documentation, and handling.
