Case Manager - Community Services
We are looking for a Case Manager who will provide comprehensive case management services to clients and their families or significant others to ensure they have access to community-based services and that all requirements from referral sources are met.
Starting salary: $47,500.
Bilingual: $49,500.

The Case Manager will assist the Case Management Supervisor in all aspects of the Youth Diversion Team Program and have a Bachelor's Degree in social work, psychology, or counseling or equivalent experience.
Key Responsibilities- Focuses on engaging clients from the time of the referral through the entire course of the client's treatment services.
- Assists clients in connecting with and gaining eligibility for entitlement programs, including public and/or private insurance programs.
- Conducts standardized screening for health‑related social needs and offers resource and referral services specifically tailored to that client.
- Provides linkage and navigation assistance for clients and their families or significant others so they can access all relevant community services.
- Helps clients and their families identify and access other Village clinical services and/or other community resources.
- Maintains and updates service data to ensure accurate reporting when required.
- Develops and maintains collaborative relationships with other community agencies, organizations, and referral sources.
- Complies with all agency policies and procedures, HIPAA regulations, Joint Commission standards and funding source requirements including complete, timely, and accurate documentation of all relevant client information as required.
- Demonstrates cultural competence necessary to serve a diverse client population.
- Participates in a collaborative team approach and engages in ongoing quality improvement efforts to enhance the functioning of the program.
- Contributes to agency visibility and development of community linkages by representing The Village in community groups or committees, when directed by Supervisor.
- Performs other duties and responsibilities as assigned by management.
Travel is required throughout the state of Connecticut. Our programs serve the following towns:
- Bolton, Lebanon
- Bozrah, Lisbon
- Chaplin, North Stonington
- East Windsor, Preston
- Franklin, Sprague
- Scotland
- Bachelor's degree required.
- At least 3 years of direct experience in SUD treatment, with familiarity with CM protocols and evidence‑based practices.
- Must be culturally competent to serve a diverse client population; bi‑lingual (English/Spanish) strongly preferred.
- Excellent oral and written skills and ability to communicate comfortably with children, parents, and both primary care and behavioral health professionals.
- Organized and able to work independently with minimal supervision. Ability to keep organized records of monetary reward systems.
- Proficient with word processing and spreadsheet applications. Possess solid general computer skills, including ability to work in a Windows environment.
