Job Overview
An opportunity has opened for aspiring RMN's to join our accredited Home Treatment Team in the heart of Harrow. The role requires a senior nurse to provide clinical direction and leadership (along with the Manager) to all staff, ensuring that patients and their carers receive safe, high‑quality mental health care as an alternative to inpatient admission. The post‑holder supports team members, develops and sustains relationships with acute inpatient services, LPS, MHCAS and community mental health teams.
The position is supported and supervised by the Harrow HTT Manager. The team works 365 days a year and staff are expected to work a mix of LDs (8‑21:00) and management days (8‑16:00) across seven days, with some flexibility.
- Ensure that good working relationships are maintained within the multi‑disciplinary team and with external agencies through effective and clear communication.
- Take a lead role in setting and maintaining standards of care within the Home Treatment Team, in consultation with the HTT Manager and Senior Practitioner.
- Provide comprehensive, culturally appropriate, evidence‑based initial assessment of people referred to the HTT, determining the specific care needs and managing risk indicators.
- Maintain an environment that promotes equal opportunity and anti‑discriminatory practice.
- Ensure safe and effective delivery of care, utilising staff skills while supporting management of stress.
- Promote a positive image of the service to users, their carers, family and partners, and liaise with all other agencies.
- Participate as an active team member, accountable for practice and working in partnership with other professionals.
- Identify Adult Protection and Child Protection issues and implement procedures as required.
- Act as Safeguarding lead, ensuring adherence to safeguarding legislation and guidance and highlighting issues to managers.
- Lead medication administration and management in line with NMC guidelines and Trust policy, educating users and carers about medication.
- Assess clients’ housing, financial, vocational and other practical needs and liaise with CMHT Care Coordinators and housing and benefits offices.
- Provide support, information and training to family members, carers and other professionals within client confidentiality constraints.
- Ensure effective and timely throughput and discharge from the team.
- Co‑ordinate shifts, prioritise work, monitor staff activity and use the team diary to maintain safety and accountability.
- Become conversant with statutory and non‑statutory services across the Trust and promote effective liaison.
- Screen and prioritise referrals to the HTT.
- Undertake initial assessments and lead development, implementation and evaluation of individual treatment plans.
- Work closely with the first response team to ensure effective gate‑keeping for referrals to acute inpatient services and facilitate discharge from inpatient care.
- Act as the Patient Flow Coordinator and support management of patient flow.
- Lead conflict resolution regarding decision‑making around appropriate care.
- Serve as an expert practitioner, providing advice and support to team members.
- Demonstrate risk assessment and management skills, practising safely according to individual needs.
- Deploy team members effectively each shift to maximise time spent with service users.
- Liaise with referrers and the Community Recovery Team, promoting continuity of care.
- Recognise the role of advocacy and promote empowerment and recovery.
- Provide education to family members, carers and professionals.
- Demonstrate clinical knowledge of therapeutic interventions for acute mental health problems.
- Adhere to the Quality Network for Crisis and Home Treatment Teams (QN‑CRHTT) Standards set by the Royal College of Psychiatry.
- Implement and maintain an effective framework for supervision within the team.
