Job Description Sentara Medical Group has an employment opportunity for a full time Nurse Practitioner or Physician Assistant to join its
** Palliative Care team
** on the campus of
** Sentara Rockingham Memorial Hospital
** located in the college town of
** Harrisonburg, VA.
** The Nurse Practitioner or Physician Assistant is responsible for providing clinical care, support, and education for the palliative patient population as part of the health care team.
** Position Highlights:
*** Monday-Friday
* Performing palliative care consultations and follow up visits for hospitalized patients with acute palliative care needs including symptom management and complex medical decision making
* Facilitating family meetings related to current medical decision making and advance care planning
* Assisting with developing appropriate discharge plans with respect to established goals of care including close collaboration with Hospice for patients to ensure access and continuity of care.
* Actively participating as a member of an interdisciplinary team to ensure that palliative care is provided in a manner which addresses pain and symptom management; psychosocial/spiritual support; and caregiver support.
