This position is responsible for leading and supporting colorectal cancer screening efforts. This role will serve as a clinical navigator, coordinating patient care, educating patients, and ensuring timely follow-up for colorectal cancer screening. The Navigator will work collaboratively with healthcare providers, patients, and community partners to reduce barriers to screening and improve outcomes. The role requires clinical expertise, cultural sensitivity, and strong communication and organizational skills.
Responsibilities
Patient Navigation & Education
Educate patients and families on the importance of colorectal cancer screening using culturally and linguistically appropriate methods.
Provide clear instructions for stool-based tests and colonoscopy preparation.
Conduct barrier assessments and develop individualized care plans to address obstacles to screening.
Care Coordination
Assist patients with scheduling and attending colorectal cancer screening appointments.
Ensure follow-up on abnormal results.
Coordinate referrals to gastroenterologists or other specialists as needed
Arrange transportation and interpreter services as needed.
Clinical Support
Utilize clinical skills to reinforce provider recommendations and ensure patient understanding.
Monitor patient compliance with screening protocols and provide reminders and follow-up.
Support patients before, during, and after screening procedures.
Data Management & Reporting
Document patient interactions and screening outcomes in the Electronic Health Record (EHR).
Maintain tracking tools and generate monthly reports in accordance with guidelines.
Ensure accurate data collection for quality improvement and program evaluation.
Team Collaboration
Partner with the clinical team to promote screening goals.
Establish referral partnerships with local GI specialists and community-based programs.
Participate in team-based care planning and contribute to clinical workflow improvements.
Compliance & Confidentiality
Adhere to HIPAA regulations and policies regarding patient confidentiality.
Follow clinical protocols and maintain professional standards in all patient interactions.
Preferred:
Licensed Practical Nurse (LPN) with current Virgina license in good standing
Medical Assistant with Associates or higher level degree in related field
Will consider individuals with bachelor’s degree in health sciences, public health, social work, or related field
Minimum of 4 years in a healthcare setting
Familiarity with appointment scheduling, patient education, and addressing barriers to care required
Experience in patient navigation, case management, or public health outreach preferred.
Experience working with individuals with difficulty accessing healthcare preferred
