Blue Ridge Health and Rehab is seeking a FT PT. The Physical Therapist (PT) is responsible for evaluating, planning, and implementing individualized treatment programs for residents in a Skilled Nursing Facility. The PT works with patients recovering from illness, injury, or surgery to improve mobility, strength, balance, and functional independence while promoting safety and quality of life. The role requires collaboration with an interdisciplinary healthcare team and compliance with all regulatory and documentation standards Clinical Care

  • Perform comprehensive physical therapy evaluations and reassessments

  • Develop individualized, evidence-based treatment plans based on physician orders and patient goals

  • Provide skilled physical therapy interventions including:

    • Therapeutic exercise

    • Gait training

    • Balance and neuromuscular re-education

    • Functional mobility and transfer training

    • Pain management techniques

  • Educate patients and caregivers on exercises, safety, and functional strategies

  • Monitor patient progress and modify treatment plans as needed

  • Ensure safe patient handling and fall-prevention practices

Documentation & Compliance
  • Complete accurate, timely documentation in accordance with facility policy, Medicare/insurance guidelines, and state/federal regulations

  • Participate in care plan meetings, discharge planning, and utilization review

  • Maintain compliance with all SNF, CMS, and therapy regulations

Collaboration & Communication
  • Communicate effectively with physicians, nursing staff, occupational therapists, speech therapists, and other interdisciplinary team members

  • Provide recommendations for durable medical equipment (DME) and assistive devices

  • Support patient-centered care and resident rights

Professional Responsibilities
  • Maintain active state PT licensure

  • Adhere to ethical and professional standards of physical therapy practice

  • Participate in quality improvement initiatives, staff meetings, and continuing education

  • Supervise and provide clinical guidance to Physical Therapist Assistants (PTAs) and therapy aides as applicable

Qualifications Required
  • Doctorate or Master’s degree in Physical Therapy from an accredited program

  • Current state Physical Therapist license in good standing

  • CPR certification (or willingness to obtain)

  • Knowledge of SNF regulations and geriatric rehabilitation principles

Preferred
  • Prior experience in a Skilled Nursing Facility or post-acute care setting

  • Experience working with medically complex and geriatric populations

  • Familiarity with electronic medical records (EMR) systems

Physical & Work Requirements
  • Ability to lift, assist, and transfer patients as required

  • Ability to stand, walk, bend, and perform physical tasks throughout the workday

  • Visual, auditory, and communication abilities sufficient for patient care and documentation
