Registered Nurse (RN) - Home Health
Location:

Pay Rate: $45-$50 per hour on W2
Job Description:
Join our compassionate team as a Registered Nurse (RN) in our thriving home Health division! In this essential role, you'll provide skilled nursing care to clients of all ages in the comfort of their own homes. As part of our interdisciplinary team, you'll coordinate care, assess patients' needs, and ensure the delivery of high-quality, personalized care.
Key Responsibilities:
- Conduct comprehensive assessments of home health and/or hospice patients, identifying their physical, psychosocial, and environmental needs.
- Develop, implement, and evaluate individualized Plans of Care (POC) to ensure continuous and top-notch care.
- Educate patients and families on various aspects of care, utilizing tailored teaching methods.
- Supervise and provide clinical guidance to support staff for outstanding service delivery.
- Act as a liaison between patients/families, team members, and healthcare professionals, ensuring effective communication and collaboration.
- Current state license as a Registered Nurse is required.
- 1-2 years of experience as a Registered Nurse in a clinical care setting is required.
- Proficiency in Homecare Homebase Software is required.
- Prior experience with OASIS is required.
- Possession of a current CPR certification is required.
- Reliable transportation and insurance are necessary.
- Graduation from an accredited Associate Degree in Nursing or Nursing Diploma program is required.
- 2+ years of home health experience.
- Bachelors of Science in Nursing (BSN) is preferred.
- CHPN certification is a plus.
