Base pay range: $44,600.00/yr - $61,400.00/yr

$5,000 sign‑on bonus available.

• Provide professional nursing services under the direction of an RN in compliance with the state Nurse Practice Act, licensed or certified requirements, and Center Well Home Health policies and procedures.
• Deliver skilled nursing interventions for patients’ illnesses, rehabilitative needs and preventative care, using a holistic approach for problem‑specific therapeutic interventions and teaching activities aligned with the plan of care.
• Apply clinical knowledge to facilitate problem resolution and achieve individualized patient goals and outcomes.
• Consult with the RN Supervisor regarding needed changes in the plan of treatment; accept verbal orders from physicians as permitted by state law and communicate these orders to the RN Supervisor.
• Implement a systematic, individualized, goal/outcome‑driven approach to the nursing plan of care.
• Maintain timely contact with patients, physicians, clinical manager(s) and other team members regarding patient/family needs and status changes; participate in care coordination activities and discharge planning as appropriate.
• Maintain the highest standards of professional conduct regarding confidential information, sharing data only when the recipient’s right to access is established and it is in the best interests of the patient.
• Communicate to ensure adherence to professional standards in the provision and availability of supplies, materials and equipment needed for safe, effective care.
• Prepare, submit and maintain documentation as required by the company and/or facility; document visit/shift notes on the day services are rendered.

• Graduate of an accredited Licensed Practical Nursing Program or School of Vocational Nursing.
• Current nursing license in the practicing state.
• Valid driver’s license, auto insurance andyanye transportation.
• Current CPR certification.
• At least two years of experience as确认 LPN/LVN in a clinical setting, preferably in home health or hospice.
