Job Summary
Harlow South Primary Care Network (PCN) have an opportunity for an experienced care coordinator to join our team. This role supports the delivery of an exciting new project and ensures patients receive coordinated and comprehensive care while maintaining high data quality standards, ultimately improving overall health outcomes. It will support coordination of the wide range of patient care we offer, including Cancer Screening invitations, proactive long‑term care for high‑risk patients, and housebound appointment coordination.
The role also includes back‑office and administrative support to ensure the smooth running of the PCN clinics.
- Provide extra time, capacity and expertise to our practice team to help improve the care we provide to patients.
- Help support upcoming initiatives, such as increasing uptake in cancer screening programmes, by proactively calling patients and increasing team capacity.
- Ensure patient health and care planning is timely, efficient and patient‑centred, and that all patients receive the best possible care and service.
- Become an important member of your PCN multidisciplinary team (MDT), identifying patients in need of proactive support and ensuring appropriate support is available.
- Work alongside a variety of people across the PCN and wider community teams, including doctors, nurses, healthcare assistants, social prescribing link workers, physiotherapists, paramedics, clinical pharmacists, and the wider administrative team.
- Provide support to the operations lead to ensure the smooth running of the PCN clinics.
- Support patients to attend services in the PCN as an addition to their GP practice services.

Our offices are based in a new purpose‑built Health Centre in Harlow. We are committed to traditional family‑oriented services and work closely in a primary care network model with Lister Medical Centre, The Hamilton Practice, and The Ross Practice.

Job Responsibilities- Patient Care:
Provide high‑quality care to patients, including telephone‑based assessments and follow‑ups. - Collaboration:
Work with social prescribing link workers and other health professionals; work closely with primary care team members to deliver the above responsibilities. - Documentation:
Maintain accurate, confidential patient records and documentation, ensuring accuracy, completeness and consistency of patient data. - Data Audits:
Participate in audits and data quality checks to identify and resolve discrepancies. - Reporting:
Support the preparation and submission of monthly, quarterly and annual claims and reports. - Meeting Coordination:
Coordinate and minute multidisciplinary team meetings (MDT). - Review Scheduling:
Invite and book reviews for a variety of patient appointments, including annual reviews. - Carer Support:
Provide signposting for carers to the social prescribing team. - Proactive Initiatives:
Take ownership for delivery in areas identified for improvement in patient care and implement solutions to enhance overall efficiency and effectiveness. - Additional Roles:
Perform any other roles deemed necessary to support the delivery of care to the practice and PCN population. - Knowledge of Regulations:
Understand healthcare data standards and regulations.
- Experience working in a primary care setting; previous experience in a patient navigation or care coordinator role preferred.
- Strong written and verbal communication skills; able to communicate professionally and effectively with a variety of people at all levels.
- Good IT/computer skills including Word and Excel.
- Motivated by helping people with care and empathy and delivering person‑centred support in a non‑judgemental way.
