Job Description

The state-supported living centers serve individuals with intellectual and developmental disabilities who are medically fragile or have behavioral problems. The centers provide campus-based direct services and supports at 13 locations throughout the state.
This role offers free CEUs and updated resources through Lippincott access for all nurses.
- Attend work on a regular basis; may be scheduled for specific shifts, rotating shifts, extended shifts, or overtime. May serve in an on‑call status on a rotating schedule as required.
- Monitors the health status of all individuals assigned to the RN Case Manager, conducts rounds, gathers information from support professionals, nursing staff, and others, and documents and follows up on issues.
- Oversees the implementation of nursing interventions, evaluates outcomes, and uses established nursing procedures to identify current and emerging issues.
- Provides hands‑on nursing care as needed and guides LVNs and direct‑care staff to implement health‑care plans.
- Participates as a member of the interdisciplinary team (IDT) as the nursing expert, proactively solves problems related to care, educates team members, the individual, the LAR, and other medical providers, and advocates for the individual’s preferences.
- Prepares for essential meetings related to individuals on the caseload, updates automated systems, and communicates information to other disciplines.
- Performs physical assessments on a monthly, quarterly, and annual basis and, as needed, develops acute and integrated health‑care plans and referrals.
- Completes required documentation (daily, weekly, monthly, quarterly, annually) in accordance with established guidelines and protocols.
- Reviews physician’s orders and Medication Administration Records (MAR) to ensure correct medication administration and identifies adverse reactions.
- Attends on‑campus medical appointments and, when required, off‑campus specialist appointments.
- Participates in competency‑based training, continuing education, and safety and infection control practices; maintains licensure as an RN and follows information‑security protocols.
- Establishes and maintains professional work relationships; mentors new RN Case Managers as assigned.
- Participates in quality assurance reviews, nursing policy development, and other work groups or committees.
- Provides treatment to HHS employees injured in the course of employment.
- Participates in disaster response, continuity of operations (COOP) activities, and may require alternate shift patterns.
