Scope of JobResponsible for medical records release of information, compiling, processing, and managing patient health medical record information generated at UT Health RGV clinics/facilities within the Electronic Medical Records (EMR) system.
Description of Duties- Organize, retrieve, and manage patients’ health information data.
- Prepare timely patient health medical record information for internal and external correspondence and requests including faxes, email requests, subpoenas, or other court orders.
- Ensure release of patient health information is complete, accurate, timely, compliant, and appropriate.
- Process affidavits and subpoenas following legal regulations.
- Ensure compliance with records management bylaws, policies, procedures, and regulatory requirements.
- Act as liaison between internal/external health center staff, physicians, revenue cycle team, health plans, and legal representatives.
- Maintain confidentiality of medical records following PHI and HIPAA guidelines.
- Monitor data integrity; research, verify, merge duplicate medical records, and correct patient overlays per established protocols.
- Meet established productivity, schedule adherence, and quality standards.
- Verify and cross‑reference information in the EMR system to ensure document accuracy.
- Prepare, scan, and index protected patient health information in electronic records.
- Track all medical record releases as required.
- Ensure patient charts/records are retained, archived, or disposed of in accordance with bylaws and regulations.
- Pull charts for other departments per request.
- Create copies and apply fees for copies as needed per department policy.
- Perform other duties as assigned.
- High School Diploma or equivalent (required).
- Associate degree in Business Administration, Health Information Management, or related field (preferred).
- Two (2) years of medical records technician experience and/or hospital or healthcare billing experience.
- Knowledge of registration, insurance verification, prior authorizations, customer service, posting, collections, reimbursement, and payment reconciliation processes.
- Proficient with Electronic Health Record system and standard office equipment.
- Microsoft Office applications – Outlook, Excel, Word.
- Strong attention to detail, excellent communication (oral and written), and organization skills.
- Knowledge of insurance plans and referral procedures.
- Comprehensive knowledge of and compliance with HIPAA rules and regulations.
Office/healthcare environment; some travel and weekend work required.
Exert up to 20 pounds of force frequently.
