As a Community Support Specialist, you’ll help individuals adapt to and thrive in their communities by creating personalized care plans that support long-term wellbeing and independence. You’ll use evidence-based strategies and collaborate with other professionals to provide comprehensive mental health services. Key Responsibilities:
  • Provide community-based mental health services including assessment, diagnosis, treatment planning, and intervention implementation
  • Collaborate with clients, families, and other stakeholders to develop and implement individualized treatment plans
  • Utilize a strengths-based approach that incorporates evidence-based practices such as Motivational Interviewing, Positive Reinforcement Techniques, Trauma-Informed Strategies, CBT, and DBT
  • Provide behavioral interventions using methods including cognitive processing, operant conditioning, and group dynamics
  • Coordinate care with external providers including physicians, psychiatrists, schools, and community organizations
  • Maintain accurate and timely documentation of services and client progress
  • Facilitate PSR (Psychosocial Rehabilitation) group sessions as aligned with treatment plan goals
  • Participate in ongoing staff development and training opportunities
  • Contribute to quality improvement initiatives through data collection and reporting
  • Build and maintain collaborative relationships with families, schools, and community supports
  • Perform additional duties as assigned

ACT/ACT TAY/IMPART Teams Only:
Apply knowledge of ACT model principles, including team-based service delivery, client-centered planning, and community-focused interventions.

Education and/or Experience

Qualifications:
  • Bachelor’s degree in Psychology, Social Work, or related human services field; or
  • Bachelor’s degree in an unrelated field and two years of qualifying experience; or
  • Any four-year combination of higher education and related experience; or
  • Associate’s degree in Behavioral Health Support; or
  • Four years of qualifying experience

Qualifying experience includes working with individuals with mental health disorders, substance use disorders, or developmental disabilities, including services such as skill-building, group or one-on-one support, recovery-focused coaching, or assistance in community functioning and employment.
