• Assess client's skills, barriers and support needs to create a pathway for employment. To include hands on community-based job skills coaching; job application and resume assistance; assistance with developing interviewing skills; on the job training/support and employment retention services.
• Maintains existing and identify new employer partnerships with internal and external businesses to identify/develop job leads and opportunities for career exploration.
• Communicates and maintains good rapport with program candidates, referring counselors, natural support of the program candidates, fellow staff and other agencies.
• Maintains program files in accordance with CARF, Ability One guidelines, DBHDS, DOL and other funding agency standards and the confidentiality thereof.
• Provides required data on persons served entering data timely and accurately.
• Serves as a liaison with referral agencies by maintaining consistent communication regarding the progress of program candidates on caseload and includes attending service planning meetings and case staffings.
• Maintains a caseload sufficient in number, achieving enrollment, placement and billable unit/hours goals per month.
• Completes and reviews service plans, billing information and case notes in a timely, comprehensive and accurate manner to reflect progress towards employment goals.
• Advocates for program candidate's needs/services by providing referrals to community resources/services to assist the program candidate with maintaining program retention and/or employment.
