Role OverviewPart-Time Psychiatric Mental Health Nurse Practitioner — Hampton, VA

- Conduct comprehensive psychiatric assessments, formulate diagnoses and develop individualized treatment plans for residents in post-acute and long-term care settings
- Provide evidence-based pharmacological and non-pharmacological interventions, monitor response and adjust treatment as appropriate
- Collaborate closely with nursing staff, primary care providers, social workers and other multidisciplinary team members to coordinate care
- Monitor medication efficacy and safety; manage side effects and provide medication education to patients and families
- Document clinical encounters and care plans accurately and promptly in the EMR
- Participate in facility rounds, case conferences and care-planning meetings to support holistic patient care
- Provide education and support to facility staff and families on behavioral health topics and best practices
- Master’s or Doctoral degree as a Nurse Practitioner from an accredited program with PMHNP certification
- Active and unrestricted Virginia Nurse Practitioner license and current DEA registration
- Experience in psychiatric/behavioral health, with geriatric mental health experience preferred
- Previous work in post-acute, long-term care or similar settings is advantageous
- Proficient with EMR systems and clinical documentation
- Excellent interpersonal and communication skills; ability to work effectively within multidisciplinary teams
- Demonstrated compassion, sound clinical judgement and commitment to patient-centered care
