Overview
Enhabit Home Health & Hospice is seeking a Registered Nurse (RN) for Home Health roles in multiple Virginia locations. This opportunity involves delivering patient-centered home-based care and collaborating with interdisciplinary teams to support patients in their homes.
Responsibilities- Perform visits and coordinate client care as the point of contact for all disciplines involved in patient care.
- Oversee the frequency of visits for the episode and consult with physicians and the office as needed, providing details about patient care.
- Perform accurate OASIS collection, maintain current medication profiles, ensure lab values are reported to the physician in a timely manner, and attend weekly case conferences and monthly case manager meetings.
- Be currently licensed as a Registered Nurse (RN) in the state of employment.
- A minimum of one year of clinical experience is preferred.
- Demonstrate knowledge and skill in current nursing practice.
- Possess a valid state driver’s license and automobile liability insurance.
